What's Needed
In
order for you to receive your card we need the following
information:
1. Provide a completed application.
2. Family name.
3. Baptismal or birth certificate showing parents, etc.
4. A picture that is 1"x1" of yourself.
5. An Elder must sign your application form.
6. Sign only (Member's signature on back) of the membership card.
7. The affidavit section of the application must be signed by a
Notary Public or Commissioner for taking affidavits as to provide of
Aboriginal Ancestry. This can be done at any municipal office.
