| Dear Interested Member,
We appreciate your taking and interest
in the Bonnechere Algonquin Community by applying for a
membership. There are a few things that we require you to do
before you can be considered to become a registered member.
You will need to provide us with a
COMPLETED application form. If it is not completed fully, we
will send it back to you. We will also need a BAPTISMAL
CERTIFICATE, this is to show us your parents names. Also any
supporting documents you may have about your family history can
be photocopied and attached to your files. We will also need a
clear (1" x 1") picture of yourself. Only sign the membership
card where it says "MEMBERS SIGNATURE" do not fill in the rest
of the member card. We will fill out your card after you have
been approved for membership.
Your application must also be signed by an
ELDER of our community. If you need names of the Elders, please
contact meat (613) 687-4141, I will gladly assist you. If you do
not know an Elder, you must obtain some information that will
prove -your claim to having native heritage.
We request all documentation with (2)
months to properly process your application. All properly
completed applications will be processed faster. Your NAME,
BIRTH DATE, and POSTAL CODE as with all of the other questions
MUST be filled out completely and clearly. Please be sure to
have your application NOTARIZED - refer to (G) Affidavit for
proof of Aboriginal Ancestry on your application form.
